Submit An Ontario Junior Citizen Nomination
Thank you for your interest in recognizing outstanding young people, age 6 to 17, who are performing worthwhile community service; are overcoming limitations (physical or psychological); have performed acts of heroism; or are ‘good kids’ who go above and beyond what is expected of someone their age.
Please read the instructions carefully and make sure each section is completed before submitting a nomination by November 30, 2017. Tip: you may wish to formulate and save your answers to each question in a Word Document first.
Required:
□ Nominator contact information
□ Nominee’s contact information
□ Nominee’s date of birth
□ Parent/Guardian Signature (VERY IMPORTANT) to acknowledge and approve the release of information in the nomination. (The form and full PIPEDA policy is available at www.ocna.org/juniorcitizen)
□ At least two supporting forms (these are like reference letters and must be completed by two individuals other than the primary nominator who are willing to support the nomination)
Optional:
□ Photos of the nominee performing acts of community service, etc.
□ Newspaper articles featuring the nominee
□ Additional letters of support
□ Links to YouTube videos or additional online media
Every nominee will receive a Certificate of Recognition from his/her local, weekly community newspaper. Up to 12 individual final recipients may be chosen as Ontario Junior Citizens and will be announced January 26, 2018 at www.ocna.org.
Please keep in mind that the award selection is based on the information you provide on the nominee. We receive many nominations each year and unfortunately, some are eliminated from the judging process due to a lack of information. Provide as much background as possible on the young person you are nominating. Additional support material regarding your nomination (such as letters of endorsement, newspaper clippings, and so on) would be helpful to the judges.
